To provide you with prompt dental treatment please be sure to bring the following when you visit our practice for your initial visit:

• Photo Identification

• Any medical records pertaining to your visit (including pre-medication requirements)

• Insurance Information/Card

Please arrive 15 minutes before your scheduled appointment time to fill out necessary paperwork.

Payment for dental care is required at the time services are rendered. For your convenience, we accept cash, personal checks, money orders, MasterCard, Visa, Discover, and CareCredit. When more extensive dental care is necessary, financing options are available to help you achieve your beautiful smile.
